If someone is credited as Story By and also Screenplay By would the Story By credit get OMB or Writer.

Writer.

I'll just edit myself here... If the story was published as a book, novel, stageplay or something like that, it gets OMB-credit.

If the story was written specifically for the movie, it gets writer. When it says "Story by" in the credits, I would be inclined to go with "Writer"...

Story by -> Original Material by
Screenplay by -> Screenwriter

The only time Writer is used is when the credit says Written by or Writer. This has been discussed extensively in earlier threads.

Yea, I don't see that in the rules.
No dictator, no invader can hold an imprisoned population by force of arms forever.
There is no greater power in the universe than the need for freedom.
Against this power, governments and tyrants and armies cannot stand.
The Centauri learned this lesson once.
We will teach it to them again.

100% correct!

Except that we are not discussing "screenplay by" but "original screenplay by".

The "only Film Credits to Include column" people should not list this crew role at all because "original screenplay by" is not listed.

The "go by function" people ("notes column") would list either "screenwriter" or "writer" depending whether the story has been written originally for this screenplay.

The "go by your heart" people (and the "go by IMDb" people too) would also list Arthur Conan Doyle as OCB, even if he's not in the credits (at least, I could not find his name there).

Just kidding! But, seriously, I think that Rule should be rephrased so that everybody reads it the same way.
Here are a few profiles in the Invelos database for that same movie:

013131-213393 US
Writing:
John Hopkins Writer
(just that; maybe because he is in the opening credits while John Loyd and Elwyn Jones are "hidden" at the end of the end credits)

8-713045-209537  Netherlands
Writing:
John Hopkins OCB
(just that)

5-050582-012859  Spain
Writing:
John Hopkins Screenwriter
Elwyn Jones Original Material By
John Lloyd Original Material By
Arthur Conan Doyle Original Characters By

5-060049-140414 UK
Writing:
John Hopkins Writer
John Lloyd Original Material By
Elwyn Jones Original Material By

I own the UK version and was wondering whether contribute a change for Hopkins from "Writer" to "Screenwriter". But the Notes in the Credits Rule make me doubtful.

Is it still the general consensus that a writing credit in a film as only "Original Screenplay" gets a Screenwriter credit? That would seem to follow the current rules.
