If we do, I reckon it is the exception to the rule and can be judged on a 'case by case' basis.

Keep the 'nickname' in the middle name slot in my humble opinion

This particular case has me thinking though, as my first instinct tell me they called him "Idaho Lewis", with Idaho obviously only being a nickname add-on. But moving it to the middle name field would indicate they only called him "Idaho"... hope this kind of makes sense to anyone. and sorry for throwing in the wrench Based on that I voed for "Idaho" Lewis//Croft.

...I am aware that this is mainly based on common sense

Besides, as mentioned above, whatever would become the common name, the "on screen name" can be put in Credited As".

What is getting the No vote from you? The wrench? The lack of fields for the Credited As name?

Yup, good point.  Since the "as credited" field is not broken into parts like the "Common name" as credited would simply be: "Idaho" Lewis Croft"

Perhaps we need to add to the Rules, that "nicknames" should be excluded from the "Common Name" field.  Or, maybe not.
