I think I'd go with what is printed on the actual disk.

The majority of series that I've seen list the disc number on the physical disc (either Disc 1 or Disc One, etc), so that's what we use in the Title field of that disc (your first example, with the Series name first).  If the physical disc shows the Episode numbers, then we enter that in the Title Field (your second example, except I would still include the Series name and the Season number).

The Episode numbers, as in your second example, are typically how the Disc Descriptions are entered in the Disc Info area.

I do the same as Srehtims with one exception, I separate the Season data from the Disc data with a colon.

Season 1: Disc 1
Season 1: Disc 2
Season 2: Disc 1
Season 2: Disc 2
etc.
