I had recently made a contribution for the new blu-ray release of The Sum of Us - UPC 887090121514 and it was rejected:

"Sources for one or more of the changes and/or additions were not submitted. Please include the sources for your changes in the contribution notes, especially for cast and crew additions."

Since I cannot view my contribution any more I really cannot tell what I did wrong.
I thought all I did was contribute front and back scanned covers showing the new UPC.
Is there a way to find out what I contributed incorrectly?
