Is there a consensus regarding regional codes on 4K/Blu-ray combo releases? While the majority of Blu-rays in such combo releases are region free, smaller studios like Lionsgate are unfortunately still using regional codes for their Blu-rays (for example A Simple Favour, Sicario). Is this to be reflected in the main profile of such a combo release?

No, since the region applies to the Blu-ray disc only (and all 4K UHD discs have no region coding) it only belongs in the separate Blu-ray Child Profile.
